DOT SAP Program Steps

Initial Evaluation
The initial DOT SAP evaluation is a comprehensive, face-to-face (or virtual) assessment conducted by a qualified Substance Abuse Professional (SAP) to determine the extent of an employee’s drug or alcohol use. During this evaluation, the SAP gathers information about the employee’s history, substance use patterns, and any contributing factors. Based on this assessment, the SAP develops a customized treatment or education plan that must be completed before the employee can move forward in the return-to-duty process. This evaluation is the first step toward regaining DOT compliance and returning to safety-sensitive work.

Education/Treatment
The education and treatment component of the DOT SAP program is designed to help employees address substance use concerns and meet compliance requirements. Based on the initial evaluation, the SAP will recommend an appropriate program that includes either education or treatment to support the employee’s progress. This step must be fully completed before the SAP can conduct a follow-up evaluation and determine eligibility for the return-to-duty process.

Follow up Evaluation
The follow-up evaluation is conducted after the employee has successfully completed the required education or treatment program. During this appointment, the SAP assesses the employee’s progress, verifies compliance with the recommendations, and determines whether they are eligible to proceed with the return-to-duty process. If all requirements have been met, the SAP provides a written report to the employer, clearing the employee for DOT return-to-duty testing. Additionally, the SAP establishes a follow-up testing plan, which includes a series of unannounced drug and alcohol tests to ensure ongoing compliance and workplace safety.
Final Steps
Once the follow-up evaluation is completed, the client will receive detailed instructions on how their employer or third-party consortium can request the testing plan. This ensures that all necessary steps are followed for compliance with the Substance Abuse Professional (SAP) recommendations. The testing plan must be formally requested and implemented by the employer or consortium to ensure proper monitoring and completion.
Once a non-positive Return-to-Duty (RTD) result is received and entered into the FMCSA Clearinghouse, your dashboard will no longer display a "prohibited" status, and you will be eligible to resume safety-sensitive duties. However, while you are able to return to work, you must still complete the follow-up testing plan as required. Successfully fulfilling all follow-up testing requirements is essential to completing the SAP program.
